Claims requirements

• Police report/ accident investigation report
• Assured’s/ Operations manager’s Affidavit of Loss, Declaring the following:

I.   Nature and ownership of assured’s business
II.  Circumstances of loss
III. Actual value of the money/ securities taken/ loss
IV. Any other information relative to the loss

• Business permit
• Sworn statement or Affidavit of hat least two (2) of Assured’s personnel
• Non-waiver Agreement
• Vouchers/ deposit slip/ cash counts/ daily sales record or other documents showing the actual amount lost/ taken by the alleged robbers.
